Community Support Program
The Community Support Program matches trained healthcare providers with adults who would benefit from additional support to meet their day to day goals. We provide care and supportive services for adults who suffer from a serious mental illness. The Community Support Program offers collaborative care model that is consumer driven. Our healthcare providers assist with activities necessary to allow individuals to live with maximum independence in the community.
Develop
We meet with the person face-to-face in a safe environment to assess their individual care needs. We talk, we learn, and we share in the development and creation of their growth care plan.
Teach
We help individuals find medical care, access education and vocational opportunities, search for employment, as well as locate, safe, affordable housing. We also assist with the transportation needs related to their growth care plan.
Support
We link the individual to the needed services based on the assessment and their growth plan. We collaborate care for those who face the biggest challenges.
Grow
We teach needed life skills, interpersonal skills, and parenting skills. we advocate for their care and we participate as their liaison with providers.
